Colonial Terrace Care Center emerged as the largest nursing facility in Mayes County for the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reports.
Records indicate that the center offered 75 beds—seven more than the county’s average as of the first quarter of 2026—and provided care for an average of 40.6 residents during that timeframe.
The CMS awarded Colonial Terrace Care Center a 3-out-of-5 overall rating, surpassing Oklahoma’s nursing home average rating of 2.7.
During the same period, Shady Rest Care Center|Meadowbrook Nursing Center|Parkhill North Nursing Home ranked as the next largest nursing facilities in Mayes County, each with 65 available beds.
As of the first quarter of 2026, Colonial Terrace Care Center maintained a zero-incident status for fines or penalties.
Altogether, Mayes County was home to four for-profit nursing facilities.
U.S. Census Bureau figures show the nation’s older adult population is rising as younger segments shrink. From 2020 to 2024, Americans age 65 and up increased by 13%, while the under-18 population decreased by 1.7%. Growth in the older adult population has been substantial over the last 100 years, from 4.9 million in 1920 to over 61 million in 2024.
According to a 2021 analysis, nearly 50% of this group, about 28 million individuals, are likely to need long-term services and supports, and more than one in three may require nursing home care during their lives.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
