The overall rating for Heartsworth Center for Nursing & Rehabilitation was 2 out of 5 in the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reported. This rating falls short of Oklahoma’s statewide average of 2.7 for nursing homes.

The facility had 146 beds available, 49 more than the Oklahoma average during the first quarter of 2026. Throughout this timeframe, staff cared for an average of 62 residents each day.

By the first quarter of 2026, Heartsworth Center for Nursing & Rehabilitation had incurred a total of $24,850 in fines and received one penalty.

U.S. Census Bureau estimates indicate the number of older adults nationally is rising, while younger populations decline. Between 2020 and 2024, the U.S. saw a 13% increase in residents age 65 and older, against a 1.7% drop in those under 18. Over the past century, the country’s older adult population has surged from 4.9 million in 1920 to more than 61 million by 2024.

A 2021 study finds that about half of individuals in this age group—roughly 28 million people—could require long-term services and supports. The report also estimates more than a third might eventually need care in a nursing home.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
